What is Spravato®?
Spravato® is the brand name for esketamine, an FDA-approved prescription nasal spray for adults with treatment-resistant depression. For this condition, it may be used alone or in combination with an oral antidepressant. Spravato® is self-administered under the direct observation of a healthcare provider in a REMS-certified treatment setting. Many patients report a significant improvement in their depressive symptoms, often noticing effects within the first 24 hours after their initial dose.
Who may qualify?
Spravato® may be considered for adults with major depressive disorder who have not experienced adequate improvement after trying at least two oral antidepressants. We provide a clinical evaluation to determine whether Spravato® is appropriate and safe for you.
What happens at a visit?
You will self-administer Spravato® nasal spray under the direct observation of our clinical team. Your blood pressure, respiratory status, and overall response will be monitored, and you will remain at the treatment center for at least two hours. You must arrange transportation home and cannot drive until the next day after a restful night’s sleep.
How often is treatment?
A typical treatment-resistant depression schedule begins with twice-weekly treatment for four weeks, followed by once-weekly treatment for four weeks. Beginning in week nine, treatment is generally provided once weekly or every other week based on clinical response and tolerability.
What are the side effects and safety of Spravato®?
Common side effects can include dissociation, dizziness, nausea, sedation, headache, increased blood pressure, and feeling temporarily disconnected from your surroundings. These are typically temporary; your care team will be there to monitor any effects during treatment.

Who may qualify

Treatment-day logistics

Questions people ask before their first visit
What is a standard treatment length for Spravato® (esketamine)?
Treatment usually begins twice weekly for four weeks, followed by once-weekly treatment for four weeks. Beginning in week nine, treatment may continue once weekly or every other week. The duration and frequency are individualized based on your response and tolerability.
What's the difference between ketamine and Spravato® (esketamine)?
Spravato® contains esketamine, one component of racemic ketamine. Spravato® is an FDA-approved nasal spray for specific depression indications and is administered through a restricted REMS program. Ketamine products are FDA-approved as anesthetics but are not FDA-approved for depression indications or psychiatric disorders.
Who should be using esketamine nasal spray?
The US FDA has sanctioned the use of esketamine nasal spray as a treatment option for individuals with depression who haven't seen sufficient results from traditional antidepressant therapies.
What does a dose of esketamine nasal spray consist of?
Spravato® is available in 56 mg and 84 mg doses. A 56 mg dose uses two nasal spray devices, and an 84 mg dose uses three devices. Each device delivers one spray into each nostril. Your clinician determines the appropriate dose.
Is esketamine nasal spray covered by insurance?
Yes! It's covered by many major insurance companies. The Clearwave team will work with you to maximize insurance coverage.
Do I have to remain on an oral antidepressant?
Not necessarily. For treatment-resistant depression, Spravato® may be prescribed alone or together with an oral antidepressant. Your prescribing clinician will determine the appropriate treatment approach. For the separate indication involving depressive symptoms with acute suicidal ideation or behavior, Spravato® is used together with an oral antidepressant.
What are the contraindications for Spravato® (esketamine)?
Spravato® should not be used by patients with aneurysmal vascular disease, or an arteriovenous malformation, a history of bleeding within the brain, or an allergy to esketamine, ketamine or any ingredient in Spravato®. Other health conditions—such as elevated blood pressure or certain cardiovascular or cerebrovascular conditions—may require additional evaluation but are not automatically formal contraindications. Our clinical team will review your medical history, medications, and blood pressure before determining whether Spravato® is appropriate for you.
